The 4-week titration protocol

The single most common cause of early liver enzyme spikes is starting at the wrong dose. A Stage 4 patient who begins at 1,500mg fenbendazole plus 1.2mg/kg ivermectin on Day 1 will almost certainly see ALT/AST elevations within 2-3 weeks. This is avoidable. Every patient — regardless of cancer stage — follows the same 4-week ramp-up before landing at their target stage protocol.

Week 1-2 (everyone): Stage 1-2 doses only. Fenbendazole 222-444mg on 3 days / off 4 days. Ivermectin 0.4-0.5mg/kg Mon/Wed/Fri. All 11 other ingredients at full protocol dose from Day 1.

Baseline labs before Day 1: ALT, AST, ALP, GGT, Total Bilirubin, Direct Bilirubin, Albumin, BUN, Creatinine, eGFR, CBC, CMP. These become your reference point — every future lab is compared against them, not against generic reference ranges.

Week 2 checkpoint: Repeat liver enzymes on Day 14. If values are within your baseline range, you're clear to advance one stage in Week 3. If ALT or AST is 1-1.5x baseline, hold at Stage 1-2 for 2 more weeks. If 1.5-2x, drop Curcumin+Piperine and EGCG for a week and maximize Milk Thistle. If >2x baseline, stop all Category A compounds and consult your physician.

Week 3-4: Advance one stage every 2 weeks. Never skip a tier. Stage 3 patients escalate to 888mg FBZ + 0.6-0.8mg/kg IVM daily. Stage 4 patients escalate to full Stage 3 protocol at Week 3-4 and only reach full Stage 4 protocol at Week 5+ after another clean lab check.

Emergency signs — stop immediately, call physician the same day: jaundice (yellow skin or eyes), severe abdominal pain, confusion or slurred speech, vomiting blood or black stools, severe swelling of legs or abdomen. Stage-specific cycling

Fenbendazole and Ivermectin are dosed differently by cancer stage. The Genesis Protocol pulls from two established repurposed-drug frameworks: the Joe Tippens protocol (conservative, cycled, well-suited to Stage 1-2 and maintenance) and the Dr. William Makis protocol (high-dose, continuous, for Stage 3-4 and metastatic disease).   • No grapefruit. Alters Fenbendazole and Ivermectin metabolism. Same goes for pomelos, Seville oranges, and tangelos.
  • 15-20g of fat at every meal where you take fat-soluble supplements. This is not optional. Without it, you're absorbing about half of what you swallow.
  • Empty stomach means empty. Turkey Tail, MCP, and Lactoferrin need 2+ hours clear of food to absorb properly.

Daily macronutrient targets

Fat: ~75% of calories

Protein: ~25% of calories

Carbs: None. This is a strict ketogenic protocol. Fat and protein only.

What to eat

✅ Eat regularly · High-fat, low-glucose

  • Extra virgin olive oil (2-3 tbsp/day). Anti-inflammatory. Carrier for fat-soluble supplements.
  • Fatty fish: wild salmon, sardines, mackerel (3-4x/week). Omega-3s and vitamin D.
  • Pasture-raised eggs and grass-fed meat
  • Avocado (½-1 whole/day). Potassium + monounsaturated fat for absorption.
  • Coconut oil and MCT oil (1-2 tbsp/day). MCTs convert to ketones quickly.
  • Leafy greens and above-ground cruciferous vegetables (spinach, kale, arugula, broccoli, cauliflower). These count as fiber, not net carb. Zero impact on ketosis.
  • Raw nuts and seeds in small servings (macadamias, pecans, walnuts)
  • Grass-fed butter, ghee, and full-fat unsweetened dairy (if tolerated)

❌ Skip · Anything that raises blood glucose

  • All carbs. No grains, no bread, no pasta, no rice, no oats.
  • All sugar. Real sugar, honey, agave, maple, artificial sweeteners.
  • All fruit. Including bananas, apples, oranges, berries, tropical fruit, dried fruit. No exceptions.
  • All starchy vegetables. No potatoes, sweet potatoes, corn, peas, carrots, beets.
  • All legumes. Beans, lentils, chickpeas.
  • Processed foods and industrial seed oils (canola, soybean, corn, sunflower)
  • Alcohol (glucose spike + liver load conflict with the protocol)
  • Grapefruit and related citrus (pomelos, Seville oranges, tangelos). Interferes with Fenbendazole and Ivermectin metabolism.
  • Soy products (may interfere with certain hormone-driven cancers)

Why fat matters (the absorption math)

Seven compounds in the protocol are fat-soluble: Fenbendazole, Ivermectin, Vitamin D3/K2, Curcumin, CBD Oil, Black Seed Oil, and Green Tea Extract. Take them without adequate dietary fat and you may absorb as little as 20-30%.

Target: 20-30g of real fat at every supplement meal.

This holds at every stage. Whether you're taking 222mg of Fenbendazole (Stage 1-2) or 1,500mg (Stage 4), the fat requirement is the same.

The keto flu (and how to skip most of it)

The transition into ketosis takes 3-7 days. If you don't pre-load electrolytes, you'll hit the keto flu: fatigue, headache, brain fog, cramps. Almost all of it is electrolyte-driven and preventable.

Daily electrolyte floor during the transition

Sodium: 3,000-5,000mg. Pink Himalayan salt or plain sea salt in water. ½ tsp in 500mL, several times a day.

Potassium: 1,000-2,000mg. NoSalt (potassium chloride) works.

Magnesium: 400-600mg. Glycinate or malate. Not oxide.

Measuring ketosis

Target range: 1.5-3.0 mmol/L blood β-hydroxybutyrate. That's the therapeutic zone. Higher isn't better.
